Mayo Post-Acute Care program coming to Menomonie

Mayo Clinic Health System in Menomonie will introduce the Mayo Post-Acute Care program on Feb. 1. This program offers transitional care in a community hospital setting for patients who require further care to recover from their illness, injury or surgery before being able to return home.

As part of the Mayo Post-Acute Care program, the patient and family work with a team of caregivers to develop a personalized care plan that focuses on meeting the patient’s medical and psychosocial needs while preparing the patient for a successful return home.

Services provided as part of the Mayo Post-Acute Care program include:

  • Skilled nursing care, such as cardiac monitoring, IV treatment, pain management, respiratory support, tube feedings and wound care,
  • Rehabilitation services, such as physical, occupational, respiratory and speech and language therapies, and cardiac rehabilitation.
  • Hospital services, such as laboratory and X-ray.
  • Social work or resource assistance, such as finding equipment and other resources needed for the return home.
  • Activities, such as leisure activities and spiritual programs.

“There is a growing demand for transitional care in the Midwest,” says Paul Horvath, M.D., site chair, Mayo Clinic Health System — Red Cedar in Menomonie. “This is why we are very pleased to provide our hospital in Menomonie as an additional and new location for Mayo Post-Acute Care to meet this growing demand and provide the care needed to this patient population.”
